Temporomandibular disorders are often multifactorial, several factors may contribute at once. Understanding what may be driving your jaw pain is the first step toward managing it effectively.
🦷 Teeth Grinding and Clenching (Bruxism)
Bruxism affects approximately 22–31% of adults during waking hours. A systematic review found that bruxism increases the odds of TMD by 2.25 times (Jimenez-Silva et al., 2023). Clenching creates sustained overload on the masticatory muscles and the temporomandibular joint.
🦴 Cervical Spine Dysfunction
The trigeminal nerve converges with upper cervical nerve pathways at the trigeminocervical nucleus. When upper cervical joints (C0–C3) are restricted, this can contribute to altered jaw mechanics and increased muscle tension (de Wijer et al., 1996).
😓 Stress and Muscle Tension
Psychological stress drives subconscious jaw clenching, sustained muscle guarding, and increased sensitivity to pain. The jaw is often the first place stress manifests physically.
💻 Forward Head Posture
Forward head position changes jaw resting posture and increases load on both the cervical spine and the TMJ. Sustained desk work, phone use, and driving all contribute.
🟠 Disc Displacement
The TMJ disc can shift, causing clicking, popping, or catching. Displacement with reduction is most common. Without reduction (jaw locks) may require referral.
⚡ Trauma or Injury
A blow to the jaw, whiplash, or prolonged dental procedures can contribute. Whiplash affects both cervical spine and jaw simultaneously. Post-traumatic TMD may develop weeks after injury.
⚠️ When jaw pain needs urgent attention
If you experience any of the following alongside jaw pain, seek immediate medical attention:
- Jaw pain with chest pain, shortness of breath, or pain radiating down the left arm (possible cardiac event – call 000 immediately)
- Sudden inability to open or close your mouth after trauma
- Significant facial swelling with fever (possible infection)
- Jaw pain with sudden hearing loss or severe ear pain
- Numbness or weakness in the face
These presentations require urgent medical assessment and are outside the scope of chiropractic care.
How chiropractic care may help with TMJ and jaw pain
A 2022 rapid review of 62 systematic reviews concluded that moderate evidence supports a multimodal conservative approach as the initial management strategy for temporomandibular disorders (Al-Moraissi et al., 2022).
The TMJ-cervical spine connection
A 2023 systematic review found that manual therapy applied to the cervical spine significantly reduced orofacial pain and improved jaw function in individuals with TMD (Correa et al., 2023). The upper cervical segments (C0–C3) are particularly relevant because of their shared nerve supply with the jaw and face.
The TMJ-headache connection
The trigeminal nerve shares convergent pathways with the upper cervical nerve roots at the trigeminocervical nucleus. Research suggests manual therapy and cervical spine exercises may help manage TMD-related headaches (Bae et al., 2023).

Jaw clicking is usually caused by the disc inside the TMJ slipping and relocating during movement. Clicking alone is common and not always concerning. If accompanied by pain, difficulty opening, or locking, it warrants assessment.
